With what hospitals is Medical Dermatology Associates affiliated?
Our office is affiliated with both Northwestern Memorial Hospital and Advocate Illinois Masonic. All our MDs are
credentialed with Northwestern and Advocate. Dr. Peters and Dr. Lio hold academic appointments at Northwestern.
What should I bring to my appointment?
In order to expedite the check-in process, we ask that you bring a government issued photo ID with your legal name, your up-to-date insurance card, pharmacy information, and payment options. Copayments are collected at the time of visit if a copayment is listed on your insurance card.
How long will my appointment take?
We understand that your time is valuable. We like to tell our patients to budget at least one hour from the time of check-in to check-out for their visit. We believe in providing all of our patients with the best care possible and being thorough with all of our patient visits. This means that we do occasionally run behind.
What is your cancellation policy?
For all general dermatology appointments, a 24-hour cancellation policy is in effect. If you cancel less than 24 hours prior to your appointment, or fail to come in for your appointment, Medical Dermatology Associates of Chicago reserves the right to issue a $50 fee.
For all cosmetic appointments, 50% of the service fee is due at the time of procedure booking. This 50% fee will not be refunded if you fail to provide 24-hour notice for cancellation or rescheduling of appointment. This rule is applicable to previously purchased and scheduled package treatments.
What if I want to speak with my physician after hours?
Our normal hours of operation are Monday through Friday, 8am to 5pm. If you need to reach our physicians after normal operation hours, simply call our main office number which will connect you to our answering service and they can page the physician on call. We do not take refill requests after hours and ask that you only contact our on-call physician after hours if you have an urgent issue.
If you have a true medical emergency, please head to the nearest emergency room or contact 911.
What is your return policy?
All products purchased at Medical Dermatology Associates can be returned within 30 days after purchase with proof of receipt.
Unfortunately our cosmetic packages are not transferable or refundable and expire within one year of the original date of
purchase.
